Glioblastoma’da eksozoma yüklü sn-38 ve etopozid ilaçlarının tedavi edici etkilerinin eksozom kaynaklı mirna’lar üzerindeki ifadesinin araştırılması

Özet
Glioblastoma (GBM), yüksek proliferatif kapasitesi, invaziv karakteri ve yapısal heterojenitesiyle santral sinir sisteminin en agresif malign tümörüdür. Cerrahi rezeksiyon, radyoterapi ve kemoterapiden oluşan standart tedaviye rağmen, yüksek nüks oranları ve düşük sağkalım süreleri mevcut yaklaşımların sınırlı etkinliğini ortaya koymaktadır. Bu sınırlılığın temelinde, kan-beyin bariyerinin (KBB) ilaç geçirgenliğini kısıtlaması ve tümör hücrelerinde gelişen direnç mekanizmaları yer almaktadır. Dolayısıyla etkili sitotoksik ajanların KBB'yi aşabilecek taşıyıcı sistemlerle hedefe ulaştırılması kritik bir gereksinimdir. Topoizomeraz I inhibitörü SN-38 ve Topoizomeraz II inhibitörü etopozid, güçlü antikanser etkiye sahip olmakla birlikte klinik uygulanabilirlikleri uygun taşıyıcı sistemlere bağlıdır. Eksozomlar, yüksek biyouyumlulukları, immün sistemden kaçabilme kapasiteleri ve KBB'yi doğal yollarla geçebilme özellikleriyle umut vadeden nano taşıyıcılar olarak öne çıkmaktadır. Ayrıca köken aldıkları hücrelere özgü mikroRNA içerikleriyle tümör mikroçevresinin aktif düzenleyicisi olarak işlev görürler; eksozomal miRNA'lar hücre döngüsü, apoptoz, inflamasyon ve ilaç direncindeki rolleri nedeniyle önemli biyobelirteç adaylarıdır. Bu tez kapsamında, T-98 ve U373 GBM hücre hatlarından izole edilen eksozomlara SN-38 ve Etopozid yüklenmiş; elde edilen yapıların miRNA profili (miR-142-3p, miR-150-5p, miR-21), hücre canlılığı (MTT, LDH), oksidatif hasar (MDA, KAT), inflamatuar yanıt (IL-1β, IL-6, IL-10) ve apoptotik belirteç ekspresyonu (p53, p21, BAX, Bcl-2, Bcl-xL, Kaspaz-3/9) değerlendirilmiştir. Tüm tedavi gruplarında apoptotik etkinlik kontrole göre artmış; en belirgin artış Exo+SN-38+Etopozit (25 μg/mL) grubunda gözlenmiştir. Aynı grupta pro-apoptotik miRNA düzeyleri en yüksek, onkojenik miR-21 düzeyi ise en düşük düzeyde saptanmıştır. Sonuç olarak, bu kombine yükleme stratejisi GBM hücrelerinde canlılığı azaltmış, oksidatif hasarı ve apoptozu artırmış, onkojenik eksozomal miRNA etkisini baskılamıştır. Bulgular, benzer mekanizmalı ajanların otolog eksozomlara yüklenmesinin GBM tedavisinde umut vadeden bir yaklaşım olabileceğini göstermektedir.
Glioblastoma (GBM) is one of the most aggressive malignant tumors of the central nervous system, characterized by a high proliferative capacity, invasive behavior, and structural heterogeneity. Despite the standard treatment consisting of surgical resection, radiotherapy, and chemotherapy, high recurrence rates and poor survival outcomes demonstrate the limited efficacy of current therapeutic approaches. The blood–brain barrier (BBB), which restricts drug permeability, and the resistance mechanisms that develop in tumor cells are among the primary factors underlying this limitation. Therefore, the targeted delivery of effective cytotoxic agents using carrier systems capable of crossing the BBB represents a critical therapeutic requirement. Although the Topoisomerase I inhibitor SN-38 and the Topoisomerase II inhibitor Etoposide possess potent anticancer activity, their clinical applicability is closely associated with the development of appropriate drug delivery systems. Exosomes have emerged as promising nanocarriers due to their high biocompatibility, ability to evade immune system recognition, and potential to cross the BBB naturally. In addition, through their cell-of-origin-specific microRNA cargo, exosomes function as active regulators of the tumor microenvironment. Exosomal miRNAs are considered important biomarker candidates due to their roles in fundamental biological processes, including cell cycle regulation, apoptosis, inflammation, and drug resistance. In this thesis, SN-38 and Etoposide were loaded into exosomes isolated from T-98 and U373 GBM cell lines. The effects of the resulting structures were evaluated in terms of miRNA profiles (miR-142-3p, miR-150-5p, and miR-21), cell viability (MTT and LDH), oxidative damage (MDA and CAT), inflammatory response (IL-1β, IL-6, and IL-10), and the expression of apoptotic markers (p53, p21, BAX, Bcl-2, Bcl-xL, caspase-3, and caspase-9). Apoptotic activity increased in all treatment groups compared with the control group, with the most pronounced increase observed in the Exo + SN-38 + Etopozide (25 μg/mL) group. Within the same group, pro-apoptotic miRNA levels were highest, whereas the level of the oncogenic miRNA miR-21 was lowest. In conclusion, this combined loading strategy reduced cell viability, increased oxidative damage and apoptosis, and suppressed the oncogenic effects associated with exosomal miRNAs in GBM cells.
